New Client Intake & Insurance Verification

The first step for our new clients is to fill out the form below so that our intake team can determine how we can help, and to verify that we accept your insurance. We will email you with more information as soon as we can. Thank you!

If you are planning on private paying, there is a question where you can indicate that on the form.

• Anthem Medi-Cal 
• Healthnet Medi-Cal 

Some of our therapists also accept these forms of insurance:

• Anthem Blue Cross
• Medicare
• Partnership Healthcare

All of our therapists accept private pay in situations where the client's insurance is not one we accept, We are happy to provide a "superbill" so that those with out-of-network benefits (PPO) may get reimbursed.  Our therapists' rates range from $100 to $200, based on the level of experience, expertise, and degree of the clinician.
